WHAT INFORMATION WILL BE COLLECTED?

In order to perform a full, effective, assessment and treatment of your horse, certain data will be required. Information regarding your name, address, email and telephone number, and your horse's history and current condition will be collected prior to treatment.

WHY WILL THIS INFORMATION BE COLLECTED?

Phone and email details are needed for contact purposes. Your address will be required in order to meet you for an appointment. Payment and billing information will purely be used for payment.

DATA STORAGE AND RETENTION

Personal information will be stored in a password protected folder on a password protected computer. Personal data will be retained for as long as needed or permitted in terms of its purpose, consistent with applicable law. The criteria to determine our retention policy is as follows:

  • All data is required to be retained for 7 years after the last treatment of your horse as specified by my insurance and governing body.

  • Where there is a legal obligation to which we are subject (for example, certain laws require us to keep records of your transactions for a certain period of time before we can delete them).

  • Where retention is advisable in light of my legal position (for example, in light of applicable statutes of limitations, litigation or regulatory investigations).

  • Your personal data is required to enable me to keep detailed records about your horse(s) and the treatments they require for my insurance company.

FILING A DISPUTE OR COMPLAINT

Please contact Reform Veterinary Physiotherapy first, using the contact details on this website, if you have any concerns or complaints with the above Privacy Policy. We will endeavour to investigate and attempt to resolve any disputes regarding use and disclosure of personal information in accordance with this Privacy Policy and applicable law.

CHANGES TO THIS POLICY

All clients will be notified via email if there are substantial and significant changes to this Privacy Policy, 30 days in advance to the changes

taking place.
